The following immunizations are required for school attendance by the State of Ohio for students entering Kindergarten, 7th and 12 grades. All of these shots must be received before school starts in August 2019:
Kindergarten (Fall 2019)
5 DPT, 4 polio, 2 MMR, 2 Varicella and 3 Hepatitis B
7th Grade (Fall 2019)
Tdap and Meningococcal (to prevent meningitis)
12th Grade (Fall 2019)
Meningococcal (to prevent meningitis) -if they had one before they were 16,
they need a second one to enter 12 grade.
These immunizations can by administered in your child’s school by Jefferson County Health Department nurses in our school based clinics. Visit the parent tab of the website and click on School Based Clinics to view and print required forms to have your child seen by the health department nurses or call the school office to request forms be sent home with your child.
School Based Clinic Hours:
Stanton – Wednesday from 11 a.m. – 1 p.m.
John Gregg – Thursday from 9 -11 a.m.
EHS – Thursday from 1-3 p.m.
If you plan to have your own physician administer the vaccinations, call your physician for an appointment before school is out. It is often difficult to get an appointment scheduled in the summer before school starts.
